The Kollner Hughes technique is used to reconstruct a loss of substance of more than 75% of the lower eyelid. It combines a tarso conjunctival flap taken from the homolateral upper eyelid and a total skin graft. The second stage releases the flap at D15. Certain precautions are described to avoid a red appearance of the free neo-edge
